Every time we went to a new doctor or specialist we had to fill out paperwork with that info on it. Tht is the only thing I could think of.

My shady brother claims there are easier ways than that. (Shady is being nice btw, although he's never done identity theft). Unfortunately, SSN are actually pretty easy to come by if you know the right people, or if you are smart. All Social Security numbers follow patterns depending on the state you apply in, and those patterns are easily accessible online. I believe you can call the credit bureaus and have a hold placed on your account for opening new accounts. You may want to look into that, so the problem doesn't get worse.
And I feel so so so bad for you! We had to do a paper return, and it took forever (as in September or October) because of lost papers. Just make sure you keep detailed records of when it was mailed, and a copy of everything!
